Contact Sonja L. RosenDr. Sonja Rosen MD, FACP, AGSF is a national leader in geriatrics. She serves as both the Medical Director and Chief of Geriatrics at Cedars-Sinai. She is a Clinical Professor of Medicine at UCLA David Geffen School of Medicine and an Associate Professor of Medicine at Cedars-Sinai. She was previously Chief of Geriatric Medicine at UCLA Medical Center, SM and Medical Director of UCLA Medical Center, SM Geriatric Inpatient Unit. Dr. Rosen completed her fellowship in Geriatric Medicine at UCLA, residency in Internal Medicine at Harbor-UCLA, and medical school at the University of Chicago Pritzker School of Medicine. She is board certified in Internal Medicine, Geriatric Medicine and Hospice & Palliative Care Medicine. She is also a Fellow of the American College of Physicians and a Fellow of the American Geriatrics Society. Dr Rosen’s research and clinical interests include safe transitions of care and safe prescribing in older persons, population health strategies in Medicare Advantage, geriatric assessment, combatting social isolation, and fall prevention. She is the author of multiple chapters and research publications in these areas, and is a national and internal speaker on her original research and geriatric models of care. Dr. Rosen developed the pre-discharge pharmacy medication reconciliation program at UCLA-SM, which successfully reduced all cause 30-day readmissions and became an Advisory Council best practice recommendation. Most recently, she helped develop the Geriatric Fracture Program at Cedars-Sinai, a model of care as an Age-Friendly Health System Committed to Care Excellence. Dr. Rosen is currently a Co-Investigator for an AARP Foundation Grant titled Leveraging Exercise to Age in Place (LEAP), which has successfully helped combat social isolation through evidence-based community exercise classes, and serves as Geriatrician Leader for a Cedars-Sinai NIA grant multicenter RCT to reduce post-hospital utilization. She is also Co-Chair of the working group for social engagement for Purposeful Aging Los Angeles (PALA) for LA County and LA City.
